- You had COVID-19 at least 4 weeks ago, confirmed by a test
- You are now having new brain or nerve symptoms (like memory problems, headaches, or numbness) that started after your infection
- You did not have dementia, Alzheimer's, Parkinson's, or a traumatic brain injury before getting COVID-19
- You were not diagnosed with a mood or psychotic disorder (like bipolar or schizophrenia) before COVID-19
- You did not have fibromyalgia or chronic fatigue syndrome before COVID-19
