- Your baby was born before 32 weeks of pregnancy or weighed less than 1500 grams at birth.
- Your baby's corrected age is less than 1 month (counting from their original due date).

- Your baby does not have severe vision or hearing problems, loss of consciousness, a breathing tube, or unstable medical condition.
