- You are diagnosed with COPD by a lung specialist at Women's College Hospital
- You are 40 years or older
- You are a current or former smoker
- You are willing to download and use the Epic Care Companion app on your phone
- You have not had a COPD flare-up requiring steroids or antibiotics in the last month
- You do not have asthma (diagnosed by a Women's College Hospital lung specialist)
