- Your child has been seen at the Children's Mercy Interdisciplinary Feeding Clinic.
- Your child was under 6 years old when they tried to stop using a feeding tube.
- Your child attempted tube weaning in the last 3 years (between 2022 and 2025).
- You speak English.
