- You are a veteran getting care at VA medical centers in Nashville, Dallas, or Gainesville.
- You were hospitalized for a heart attack, a stent procedure, heart valve surgery, bypass surgery, or heart failure.

- You do not have severe memory or thinking problems.
- You do not have a life-threatening heart rhythm issue.
