- You are part of the Mobile Health and Wellness Program
- You speak English and are 18 or older
- You have a condition like high blood pressure, high cholesterol, type 2 diabetes, pre-diabetes, arthritis, obesity, heart failure, or COPD
- You take medication for at least one of those conditions
- You can attend at least half of the 24 team visits
