Who can take part
- You are 21 to 70 years old
- You identify as Black/African American or Hispanic/Latino
- Your systolic blood pressure (top number) is 130 or higher at screening, and you also have high blood pressure, prediabetes/type 2 diabetes, or stage 3 kidney disease
- You live in a neighborhood in certain Maryland counties where it's hard to find healthy food, many households have low income, and many don't have a car or are far from a supermarket
- You have a refrigerator, a microwave or stove, and a cell phone to use the study app
- You are not pregnant, breastfeeding, or planning pregnancy, and you don't have a serious medical condition like cancer or kidney failure on dialysis
