  • You live in the Marche region of Italy and are a patient at one of the listed diabetes centers.
  • You are 18 or older and own a smartphone with internet access.
  • Your most recent HbA1c (average blood sugar over 3 months) was above 7%.
  • You have not had any major health events (like heart attack or stroke) in the last 6 months.
