- You are 18 years or older
- You have taken opioids for chronic pain for at least 6 months
- You have taken at least 50 morphine milligram equivalents daily for 60 days or more in the past year and a half
- You are able to speak and read English
- You have not been diagnosed with cancer (except some skin cancers) in the past 5 years
- You have not had surgery or a psychiatric hospital stay in the past year
