Who can take part
- You are a primary care patient who has been taking an opioid for more than 3 months.
- Your daily opioid dose is more than 90 morphine milligram equivalents (MME) per day.
- Or you take both an opioid and a benzodiazepine (like Xanax or Valium).
- Or your opioid dose has been reduced by more than 5% per day recently.
- You are between 12 and 89 years old.
- You do not have an active cancer diagnosis, are not in hospice or palliative care, and do not have sickle cell disease.
