  • You are 65 years or older.
  • You have a current diagnosis of depression or bipolar disorder.
  • You are seeing a psychiatrist regularly.
  • You have at least two ongoing physical health conditions (like diabetes, heart disease, or arthritis).
  • You live on your own at home in the Leuven area.
  • You also have a regular family doctor (general practitioner) in addition to your psychiatrist.
