- You have not had varicocele repair surgery before
- You have not had any testicle problems like undescended testicle, infection, injury, or twisting
- You have not had chemotherapy, radiation, or other treatments that can harm sperm
- You have no serious untreated health conditions or genetic disorders
- Your sperm count is at least 5 million per milliliter
