This trial tests a combination of three medications (an endothelin receptor antagonist, a PDE5 inhibitor, and sotatercept) as the first treatment for people newly diagnosed with pulmonary arterial hypertension (PAH). It aims to see if starting with all three together works better than the usual approach.

Who can take part
- You are between 18 and 70 years old.
- You have been diagnosed with PAH (or PAH from a corrected heart defect) within the last 12 weeks and haven't taken PAH medications for more than 6 weeks.
- At the time of your diagnosis, a right heart catheterization showed certain high pressures in your lungs (PVR >5 WU, mPAP ≥35 mmHg).
- Your blood counts for platelets and red blood cells are normal.
- If you can become pregnant, you must use highly effective birth control during the study and for 4 months after treatment ends. If you are a man with a partner who can become pregnant, you must use condoms and inform your partner(s) of the study.
- You do not have other types of pulmonary hypertension, significant lung disease, left heart failure, a history of severe bleeding, or kidney problems (eGFR below 30).
