Who can take part

  • You are 18 years or older and are a caregiver for an adult who just visited the ER.
  • The patient you care for will go home within 72 hours.
  • You help the patient with daily tasks like shopping, doctor visits, or medicine.
  • You have a smartphone, computer, or tablet with internet.
  • You speak and understand French.
  • You are not currently in a hospital or nursing home, and the patient is not getting home-based hospital care or hospice.
