Who can take part

  • You are 18 years or older
  • You have type 2 diabetes or prediabetes with an A1c of 9.5% or higher
  • You are a patient at the Community Care Clinic and have no health insurance
  • Your household income is at or below 250% of the federal poverty level
  • You live in Forsyth County, North Carolina, or nearby areas (specifically zip codes 27101, 27105, 27107, or adjacent service zones)
  • You can speak English or Spanish and are willing to give informed consent
