- You are 18 years old or older
- You have been diagnosed with high blood pressure, with readings of 140/90 or higher, and you take at least one blood pressure medicine
- You scored 33 or below on a health literacy test (which measures how easy it is for you to understand health information)

- You can read and speak Vietnamese
