Who can take part

  • You are 18 or older
  • You have symptoms of depression (like feeling down or little interest in things)
  • You have a history of coronary heart disease (e.g., heart attack or chest pain from blocked arteries)
  • You receive care at a clinic connected to Columbia/NYP, and you see a primary care doctor or heart doctor there
  • You speak English or Spanish
  • You have not had a severe mental illness like bipolar disorder or schizophrenia, and have not had a recent integrated mental health visit
