- You are between 18 and 84 years old
- You get your blood pressure care from a doctor at Northwestern Medicine
- Your recent clinic blood pressure was high (140/90 or above, or 150/95 or above)
- You have or can get a patient portal account from Northwestern Medicine
- You do not have dementia, kidney failure, or are in hospice, and are not pregnant
