  • You or your loved one has been diagnosed with Alzheimer's disease (any stage).
  • You are 60 years old or older.
  • You have a family member or friend who helps care for you (a caregiver).
  • You live at home, not in a nursing home or assisted living facility.
  • You are not under any legal guardianship or conservatorship.
