- You have been diagnosed with inflammatory bowel disease (IBD) such as Crohn's or ulcerative colitis that is currently active.
- You have a poor quality of life because of your IBD (measured by a questionnaire).
- You have a high level of inflammation measured by a stool test (fecal calprotectin over 250).
- You can speak and read English.
- You have regular internet access through a phone or computer.
- You do not have an ileostomy, colostomy, or other similar surgical pouch, and you are not scheduled for immediate surgery.
