  • You have been diagnosed with Parkinson's disease within the last month
  • You are 18 years of age or older
  • You are not currently living in a long-term care facility or nursing home
  • Your area is served by a specialized neurodegenerative disease team
  • If you have a primary caregiver, they may also participate (but it's optional)
