- Be 18 years old or older

- Recently discharged from UAB Hospital or its emergency department
- Receive your primary care at Cooper Green Mercy Health Services or UAB Post Discharge Clinic
- Be identified as high-risk for diabetes complications in the next 3 months based on your health history
- Be able to understand and sign consent to participate
