  • Be 18 years old or older
  • Have type 2 diabetes (confirmed by blood sugar test or medical records)
  • Recently discharged from UAB Hospital or its emergency department
  • Receive your primary care at Cooper Green Mercy Health Services or UAB Post Discharge Clinic
  • Be identified as high-risk for diabetes complications in the next 3 months based on your health history
  • Be able to understand and sign consent to participate
