Who can take part
- You are between 40 and 75 years old
- You have high blood pressure and at least one other heart risk factor (high blood sugar, type 2 diabetes not on insulin, high cholesterol, or are a smoker)
- You are overweight or have mild obesity (not severe)
- You have not had a heart attack, stroke, or heart failure
- You do not have advanced kidney disease
- You are able and willing to use digital tools (smartphone app, tablet, or computer) for 24 months
