  • You live at home (not in a nursing home, hospital, or assisted living facility)
  • You are frail or at risk of becoming frail (experiencing weakness, slowness, or low activity)
  • You can hear well enough to talk on the phone and understand conversations
  • You can read, speak, and understand Turkish
  • You are willing to sign a consent form and participate in phone visits
