  • You are 18 years old or older.
  • You have been diagnosed with a serious, advanced illness.
  • You are currently getting palliative care at an outpatient clinic or through home care services.
  • You can fill out questionnaires yourself.
  • If you cannot fill out the questionnaires, a family caregiver who is 18 or older may help.
