This study aims to help people with advanced HIV who are in the hospital get back into regular HIV care after they go home. It tests a program of support to help you stay healthy and connected to treatment.

Who can take part
- You are living with HIV.
- You have advanced HIV, meaning a CD4 count below 350 or a recent infection that shows a weakened immune system.
- You are currently staying in the internal medicine department of the hospital in Lusaka, Zambia.
- You plan to stay in the Lusaka area after leaving the hospital.
- You are able to give consent and have access to a telephone.
- You do not have other serious conditions like advanced cancer (other than Kaposi sarcoma), end-stage kidney or liver disease.
