- You are a patient at Dana-Farber Cancer Institute with a DFCI medical record number.
- You are 18 years old or older.
- You are admitted to the solid tumor medical oncology service at Brigham and Women's Hospital.
- Your doctor has estimated your risk of dying in the hospital is 25% or higher.
- You were admitted between Sunday after 4pm and Friday at 4pm (excluding holidays or staff shortages).
- This admission was not planned in advance (like for a scheduled chemotherapy treatment).
