Pharmacogenomic Testing for Medication Management

This trial checks whether using a genetic (DNA) medication test—done within the last 120 days—can help reduce bad medication reactions and prevent readmissions to the hospital. It may help people whose medications have caused side effects or did not work well.

Who can take part

  • You are 25 years old or older and can sign informed consent to join the study.
  • You had genetic testing for medication-related genes within the last 120 days.
  • At the time of that genetic test, you were taking at least one medication (including over-the-counter) that can vary by genetics.
  • In the 24 months before the genetic test, you had at least one significant medication side effect, or a medication did not work well enough.
  • You are not currently in the hospital and can provide an accurate medical and medication history for the prior 120 days.
