Who can take part
- You are at least 22 years old.
- You were recently admitted to the hospital for worsening heart failure, with signs on chest X-ray and treatment with IV water pills (diuretics) or IV heart medicines (vasoactive drugs), and your BNP or NT-proBNP blood test is high.
- You can give informed consent (you may need to speak English, Spanish, or Creole).
- You have reliable cellular (mobile data) coverage where you live.
- If you had weak pumping function before (LVEF under 40%), you must have been on standard heart medicines for at least 90 days before enrollment.
