Who can take part
- You have a diagnosis of heart failure
- You have either an abnormal echocardiogram or a B-type natriuretic peptide blood level above 100 pg/mL
- Your heart failure is in Stage B through D, and your symptoms fall in Class I through IV
- You can understand and sign consent, and you agree to follow the study schedule
- You can use a smartphone and can attend follow-up visits regularly
- You have not had a heart transplant and are not using a mechanical heart support device
