  • Be a man age 35 to 89
  • Have a diagnosis of prostate cancer
  • Live in Washington state (for the registry part of the study)
  • Agree to genetic and other testing and sign consent forms
  • Be willing to fill out questionnaires and provide a saliva sample
  • Have a U.S. mailing address (or, in one group, you can also provide email or phone)
