  • You are likely to be discharged home (not to another hospital unit).
  • You are age 60 or older (or a caregiver will help you).
  • You have at least 3 long-term health problems (chronic conditions).
  • You (or your caregiver) can speak and read English well enough to use the program.
  • You can join the study and consent; mild memory problems are okay if you can participate with help.
  • You have not already joined this same program during a prior hospital stay.
