Who can take part
- You are 18 years or older
- You are within 10 days of leaving inpatient rehabilitation at one of two specific hospitals in Switzerland (Bern Rehab Center or Insel Hospital)
- You have one of these conditions: recent heart attack or heart failure, COPD (chronic lung disease), fragility fracture like hip fracture, or mild stroke
- You can understand information and agree to participate in the study
- You own or can access a smartphone, tablet, or laptop with internet connection
- You can use digital devices on your own without help, and you don't have serious language or thinking difficulties that would prevent app use
