Who can take part

  • You live in the Västra Götalands region (VGR) and are registered there
  • You are 75 years old or older and live in your own/community home (not in a hospital)
  • A primary care or emergency visit has screened you as “frail”
  • You are not in the final stage of life and do not need palliative care right now
  • You have a registered address
  • You are not already taking part in another conflicting randomized (research) study and you are not disoriented (know who/where/when)
