  • You are 40 years old or older
  • You have been diagnosed with acute heart failure by a heart specialist and hospitalized
  • You had symptoms like shortness of breath, tiredness, or swollen ankles, plus signs like congestion on chest X-ray or high BNP levels
  • You are considered frail based on a special assessment
  • You can follow instructions for simple physical tests
  • You live in Korea and understand Korean
