  • You receive care from a primary care clinic that is part of the study
  • You are 18 years or older
  • Your BMI is above 25 (a common cutoff for being in the overweight range)
  • You are able to take part in a weight-loss program (for example, you understand and can follow the plan)
  • You are able to participate from home (not living somewhere else that limits participation)
