  • You are 65 to 85 years old
  • You have a body mass index (BMI) of 30 or higher
  • You have at least 2 chronic health problems that need treatment (like diabetes, high blood pressure, or arthritis)
  • You live independently at home (not in a nursing home or assisted living)
  • You can speak English and pass a quick thinking and memory test
  • You get permission from your primary care doctor to join
