Who can take part

  • Be 21 years of age or older
  • Have a prognosis of about one year or less, based on an algorithm used by Clover Health
  • Be able to speak English or one of the other supported languages in the video program
  • You must be able to see well enough for the video (better than 20/200 corrected)
  • You should not have a sudden severe mental health crisis or other condition that would make it hard to understand and participate in the planning discussion
