  • You are 21 years old or older and live in Singapore.
  • You have a main caregiver who helps make medical decisions for you.
  • You can speak and understand English, Chinese, Malay, or Tamil.
  • You have a brain tumor, a spontaneous brain bleed, or chronic kidney disease stage 4 or 5.
  • Your doctor agrees that planning your future care is right for you.
