Who can take part
- Be the parent or legal guardian who is the child’s main caregiver
- Caregiving for a child ages 18 or younger
- Your child has serious ongoing medical needs (for example, multiple organ/health systems involved, worsening illness, or ongoing medical technology like a feeding tube, breathing equipment, or central line)
- Your child is/ will be discharged home and will need at least one medication each day
- You can speak English or Spanish and are willing/able to do the study visits or steps, including random assignment
- You (the caregiver) are 18 years or older and have not joined this study before
