  • Your child has seen at least two specialists in the past year.
  • Your child is under 18 years old and is in the hospital on a general children's floor.
  • Your child will be discharged to home (not a nursing home or rehab).
  • You (the parent or caregiver) are 18 years or older.
  • You speak English or Spanish.
