Who can take part

  • You have confirmed adenoid cystic carcinoma (ACC), and it has spread to distant sites seen on scans (biopsy of the spread is preferred but not required).
  • You have 1 to 5 metastatic spots total that are suitable to treat with SBRT (focused radiation), or—if in the other group—at least one spot suitable for another local treatment.
  • Your brain/spine situation must allow treatment: brain spots up to 3 cm are allowed, and you’re not eligible if your case needs urgent surgery for metastatic brain/spine disease.
  • Your age is 18 or older, and your overall condition is good enough (ECOG 0–2).
  • If you’ve had cancer treatment already, it can’t include chemotherapy or targeted pills within 2 weeks of starting radiation, and you must be off those drugs during the SBRT period.
  • You can give informed consent, and if you can become pregnant, you must have a negative pregnancy test and agree to contraception during and shortly after treatment.
