Who can take part

  • You have had at least 2 COPD flare-ups in the past 12 months
  • A breathing test or scan shows COPD (FEV1/FVC below 0.7, or emphysema seen on imaging)
  • You have a smoking history of more than 10 pack-years
  • You do not have severe mental illness that is not controlled by medication
