Who can take part
- Your colon or rectal tumor is adenocarcinoma and has MMR deficiency (MMR-d) or high MSI (MSI-H)
- You are age 18 or older and can sign written consent
- Your cancer stage fits one of these surgery-eligible situations: node-positive T1-4, or node-negative but “high risk” T3 (depth ≥5mm or EMVI on imaging) or node-negative T4
- Your team does NOT plan radiotherapy for you, and you do not have spread to distant sites (M1)
- Your blood, kidney, liver, and clotting test results meet required minimums
- You have not used prior immune-checkpoint drugs like pembrolizumab or similar in the past, and you are not on strong immune-suppressing medicines
