New combo therapy before surgery for low rectal cancer

This trial tests a new approach for people with locally advanced low rectal cancer that is pMMR type. You would receive a combination of an immunotherapy drug (anti-PD-1) with chemotherapy (FOLFOXIRI) and radiation before any surgery, to try to shrink the tumor as much as possible.

Who can take part

  • Your rectal cancer must be a type called pMMR (all four MMR proteins are normal).
  • The lower edge of the tumor must be 5 cm or less from the anal opening (this is called low rectal cancer).
  • The cancer must be locally advanced (stage cT3-4N0M0 or with positive lymph nodes, but not spread to distant organs).
  • You must not have had any previous chemotherapy, radiation, immunotherapy, or surgery for rectal cancer.
  • You need to be between 18 and 75 years old, with good overall health (ECOG score 2 or less) and normal organ function.
  • You cannot have other serious medical conditions like active infections, autoimmune diseases, or uncontrolled high blood pressure.
