This trial studies whether removing lymph nodes in the upper abdomen (hepatoceliac lymph nodes) improves outcomes for people with stage III or IV ovarian, fallopian tube, or primary peritoneal cancer that has spread to those lymph nodes. It also looks at how this surgery works for people with certain types of relapse, often alongside chemotherapy.

Who can take part

  • Be 18 to 75 years old
  • Have confirmed epithelial ovarian, fallopian tube, or primary peritoneal cancer (not low-grade or mucinous)
  • Have cancer in the liver/upper-abdomen lymph nodes seen on scans, and surgeons must believe it can be fully removed
  • If this is a first treatment situation, you can have had 1 to 3 rounds of neoadjuvant chemotherapy (if it has not progressed)
  • Be “platinum sensitive” if the cancer has come back (platinum-free time of 6 months or more)
  • Be well enough for surgery and chemo: good blood counts and organ function, and physically able to do daily activities (ECOG 0–2)
