  • You are 18 years or older
  • You have a doctor diagnosis of a long-term condition such as diabetes, high blood pressure, heart disease, cancer, kidney disease, or chronic lung disease
  • You visited a hospital or outpatient clinic at least once sometime between 2010 and 2024
  • You do not have a doctor-diagnosed COVID-19 (during the study period)
  • If this part of the study applies to you, you can communicate in Chinese or English and agree to participate
