Who can take part
- You’re an adult with chronic kidney disease (kidney function is quite low, including people on hemodialysis).
- You can sign consent and take part in the virtual program.
- You are currently followed by St Joseph’s Hospital Hamilton Kidney Care Clinic or a dialysis program.
- You had a recent hospital admission and are being discharged back to your usual community living situation (not long-term care/rehab/complex care).
- You can speak and understand English well enough to participate, and you have the vision/hearing needed to use the technology.
- You do not do peritoneal dialysis or home hemodialysis, and you are not a functioning kidney-transplant patient.
