- You are 18 or older.

- You have been in the hospital for more than 2 days.
- Your hospital stay has a higher chance of needing extra care (based on the LACE score).
- You live in the Calgary or Edmonton area (not outside Alberta).
- You are not on dialysis for kidney failure.
