- You’re age 18 or older
- You were hospitalized at the study sites using Alberta Health Services’ electronic health record (AHS EHR)
- You had an acute kidney injury during the hospital stay (mild to severe stages 1–3)
- You were not already a kidney transplant patient and are not a kidney donor
- Before this hospitalization, you did not have very advanced kidney disease or need dialysis
